Form Processing
A standard service charge of $10 is due upon submission to process any WCM ophthalmology form. Form processing generally occurs within seven to 10 business days, but may be expedited by an applicable physician (to be completed on the same or next business day) for $50. Consult a WCM ophthalmology staff member concerning the accommodation of expedited service if and when you require it.
School/Camp Form Policy
Plan ahead and ensure that your child has recently visited our office (within the past year) to obtain completed school or camp medical forms. We are able to complete forms based on your child's last appointment if it occurred within the past year. If your child requires a new appointment, bring any school or camp medical forms with you for completion.
