What to Bring to Your First Visit

To help us serve you better, we ask that you bring to your first visit the items listed below.

  1. Please print and complete the patient questionnaires that you can download from the Patient Profile and Registration Forms page. Completing these forms prior to your appointment will help the physician and the staff see you in a timely manner.
  2. Be sure to bring your insurance card.
  3. If you wear contact lenses or eyeglasses, please bring them with you. It is also helpful if you also bring the box your contact lenses come in. This provides us with information in regard to the brand, prescription, base curve, and diameter of the contact lenses you are currently wearing.
    Please note: If you need or wish to have your glasses prescription evaluated, please be aware that this is not included in your examination. We are delighted to provide this service as well, but it is separate with an additional $75.00 dollar fee which is not covered by your insurance carrier.
  4. If you have a prior eye problem, we recommend you obtain your previous ophthalmic records and bring them with you on the day of your visit.

About Referral Forms

Many insurance companies require a completed referral (electronic or paper form) prior to the examination, and the patient is responsible for obtaining this referral. Patients are encouraged to contact their insurance provider to comply with the required process for their specific carrier. If a referral is required by your insurance carrier, please fax it to (646) 962-0609, well before your appointment date. You may verify that it has been received by contacting the Call Center at 646-962-2020. In exceptional, urgent situations, it may be possible to bring the referral form with you on the day of your visit. However, please understand that, in accordance with Weill Cornell Physician's Organization requirements, we regret that we are unable to see patients for examination without the appropriate referral, even if it has been obtained but is not available or cannot be verified at patient check-in.
